We are still offering fax and mail services as usual, however, we are now also offering most assessments online. Send an email to info@octs.net with the following information and we can email your client a dedicated link to take an assessment online. We will receive a notification once the client completes the assessment online and then we can email you a computer generated report. Same reports, same costs, same great service!
Your email must include
- Client first and last name
- Client date of birth
- Client gender
- Client email
- Name of assessment (MCMI-III, MMPI-2, etc.)
- Name of report (Pearson Interpretive, Profile, etc.)

FILL OUT
Fill out the USER QUALIFICATION FORM and Press Submit. If you need testing materials, go to CONTACT US and let us know what you need.
RETURN
The completed answer sheets can be returned to us to be scored by fax, mail, or email. A completed Order Form must be included with the desired report checked and method of payment (billing is available for returning customers). First time faxed answer sheets require a same-day check in the mail or Visa/MC/American Express number on fax, or call it in to our secure answering service.
